Director of Bereavement Psychosocial & Spiritual Care

Haven Hospice

Experience: Minimum of five years related clinical experience in varied multidisciplinary settings.
Education: Master's degree in Social Work with an LCSW.
Salary Range: $50,600 - $63,300

As a member of the Leadership Team, the individual in this exempt, full time position will be responsible for developing, implementing, and supervising the delivery of bereavement care programs consistent with the core values and policies of Haven Hospice. This individual will
also be responsible for the clinical supervision of Social Work and counseling staff and interns as well as the spiritual care and psychosocial service processes and program development throughout the network.

Palliative Care Clinical Nurse Specialist  

St Joseph Health System                                                                                                                                            Sonoma County, CA

 

St. Joseph Health System in Sonoma County California consists of a  245 bed Level II Trauma hospital in Santa Rosa, CA and a 70 bed acute full service hospital in nearby Petaluma, CA

Our palliative care program includes a 17 bed swing medical/palliative care unit and a Medical Consult Team including a palliative certified physician and an advanced practice nurse. The Consult Team is three-years old and is in the development and program growth phase. The APN position includes all domains of the clinical nurse specialist; experience and excellence in palliative clinical practice, education of staff as well as patient/families, evidenced based research, consultation and collaboration with management of both hospitals, and continued program development.
